An overview of the procedure

All incisions are made inside the mouth, except perhaps for small incisions on the cheeks. The upper and lower jaws are freed with the use of thin saws and chisels, and they are advanced together, with placement of titanium plates and screws to hold the jaws in position. The surgery involves placement of large braces (arch bars) on the teeth that remain in place for 6 weeks; typically the jaws are not wired together, although rubber bands are usually placed to assist in maintaining a stable bite during the healing process. Patients are on a soft diet and avoid chewing for 4-6 weeks. Some patients may benefit from placement of a temporary tracheostomy to avoid problems that can occur with swelling after surgery.
This procedure can be performed in patients who have abnormal jaw positions but who do not have OSA; in OSA patients, jaw advancement surgery is different, involving greater movements of the jaws, being technically more difficult for the surgeon, and having more risks and a more difficult recovery for patients.

Sounds awful, but I'm sure it beats long-term sleep apnea. I hope it works for the big guy.

A maxillo-mandibular advancement procedure is a pretty intense step to take in the treatment of OSA. Last resort after you've failed all the routine therapies. Big surgery, painful recovery. Not compatible with having an off-season of intense strength workouts and getting the nutrition necessary to support those. The guys at Stanford are the gurus of this procedure, wouldn't be surprised if he ends up out there.

"I've been dealing with sleep apnea for a long time, my whole career," Napoli told WEEI.com in a text message. "I've tried numerous things and none of them worked. Dental mouth piece, CPAP machine, medicines ... It's just gotten to the point where I have to get this done."

I wonder if this means we see a lot of Allen Craig for the first month or two of the season. No idea how much of an impact the 4-6 weeks of a soft diet will have on his overall strength, and how long it'll take for him to get back to his normal game shape.

Tough blow, and that surgery looks gruesome, so this must be a last resort for Napoli.  For his personal welfare, I hope it goes smoothly.  Can't imagine this is how he was planning to spend the offseason.

My god that looks awful.  Clearly a last resort.  Not a great way to spend the offseason - hope he comes back strong and well rested.

Hey, they'll have to shave him pretty clean, huh?

Finally.  It took 15 posts to get to the real issue at hand?

I mean, if he has been able to be an .800 OPS guy without proper sleep for all this time...  What data do we have on players undergoing this procedure, and their performance the following season?

(mostly kidding)

This explains the Facetiming screen shot that showed up with Clay's wife. He was just getting a consultation.

I know this was already addressed, but I just can't leave it alone. We've got a professional athlete with millions of dollars at his disposal in a town with top-notch medical care who is undergoing a procedure that will literally rearrange his face in order to correct this problem (enduring an extremely unpleasant recovery in the process), and not only did the C-Pap alternative cross your mind, as if there was any chance at all it hadn't been considered already, but you actually typed it out.

Why doesn't he just sleep better? Has he specifically tried that?

What about closing his eyes?

Maybe he can try some Benadryl or Tylenol PM.  Those always do the trick for me.

I think he can put back a few 12-packs and be okay in no time.

On a serious note, the issue has to be affecting him pretty profoundly if he's willing to go through that procedure.  That looks really awful.

I'm having that surgery, hopefully this fall/winter. It will make a huge difference in my ability to breathe and it may cut down on my migraines considerably.

A close friend had a similar procedure, and it really changed his life for the better.

Bon courage à toi et Nap aussi.

Wow. This thread already has an intoxicating blend of sweet, sour and salty.
And I'm guessing Nap has already tried the "blow on the head", so it's time for thin saws and chisels.

If EV was still around he would have identified the underlying sleeping problem here months, if not years, ago through detailed analysis of home vs road stats.

Or he'd at least have proposed that Napoli try a CPAP.

Read the comments section of the new Fangraphs article about the Sox prospects that went up yesterday (great article BTW) and you will find some instant classic comments by one Eric M. Van.

I had a similar surgery when I was 17 for orthodontic reasons.  It was fucking awful.  I lost 30 pounds in like 2 weeks and my face swelled to look like Jabba the Hutt.  And here's the kicker, I have apnea now.

Just for the record, I have never been paid to love cats, so I do not think I am a professional cat lover. Just want to put that out there.

Also, yes I have tried a cpap. My problem is that my mouth is so small (I know, the irony, right?) and my airway is so narrow that it just makes me feel like I am being suffocated. The surgery will give me about a half inch or more of airway and give me more room in the sinuses. Breathing will be much easier.

I have a call into Nap's people about us sharing a room, maybe doing a "buy one, get one half off" deal.
