Probably Bronson Arroyo, given that he would have been the starter operating with most rest (2 full days!), only pitched 2 innings in his last outing, and was the first out of the pen in the real game 6. Wakefield would have seen action too, which could have been scary given his role in the 2003 clincher. Even scarier, they probably would have had to take a long look at Ramiro Mendoza, given the absence of other options.

Mendoza was a possibility.  Scott Williamson was also not on the ALCS roster, nor was Terry Adams.  They might have had to DL Schilling (and lost him for the WS) and activated one of those three.

Not sure if that was the rule then or not, but in my memory it was.  Williamson missed the bulk of the summer but had ten or so good outings in September.
 
Kim had 4 outings in late September/early November.  2 miserable ones, 2 good ones after missing all of June, July and August. 

I'm not sure Astacio was on the roster in time, his first appearance in the bigs that year was on September 8, and he had 4 terrible appearances (out of 5) in his total Red Sox run.

I think Williamson came back at the end of the year, but was still visibly impaired. What a bullpen it could have been!
