They spend hours on the course during non-playing time, in the mornings, during practice rounds, all week, marking off distances.

Edit: So for example, they'll know it's X amount of yards to a bunker or whatever, and then they'll just mark it off from there to the ball, etc.
They have the coverage of him, don't worry. Every hole is covered.
Part of the preparation for caddies is they walk the course (in the week before the tournament) and create their own yardage books using benchmarks in the fairways and around the greens to allow them to give fairly precise yardages for every shot.
